UPLIFITED is an 8-week support programme for adults over 18 who are living with a life-limiting illness and have a Buckinghamshire GP. It is run by palliative care nurses at Butterfly House and is designed to support people at different stages of their illness in a holistic, person-centred way.
The programme combines:
- nursing support
- complementary therapy
- creativity and movement
- yoga, mindfulness, and meditation
- practical advice and coping strategies
Each week follows the theme of UPLIFTED, focusing on areas such as:
Person: person-centred care and who you are beyond your diagnosis
Love yourself: self-love, self-care and exploring body and identity
Identity: navigating, a shift or loss of identity, embracing a new identity and finding opportunity in rediscovery
Feel good: feeling good mentally and physically
Trust: trusting ourselves and establishing trust in others, planning for the future
Empower: finding personal empowerment and empowering others
Develop: celebrating and reflecting on your achievements, and ways to further develop your new skills
It also helps participants with:
- managing symptoms
- emotional support
- confidence and self-awareness
- advanced care planning
- community support and advocacy
- useful worksheets and resources to continue after the course
To join, a person should be mobile enough to attend Butterfly House and able to take part in the weekly sessions. They can be referred by a GP, consultant, specialist nurse, or other healthcare professional. Before joining the Uplifted programme, patients complete an eight-week assessment conducted by the nursing team. Those who are deemed suitable are then referred to the complementary therapy team.
